Physical Information
SCHOOL PHYSICALS FOR FALL EXTRA-CURRICULAR ACTIVITIES AND WORKING PAPERS WILL BE OFFERED AT CNS ON THE FOLLOWING DATES: Physicals will be offered in the CNS HS Health Office on Thursday, May 27th for boys and girls. The yellow physical form may be picked up in the Health Office. The completed Health History side must be completed and signed by your parent/guardian----then you must sign up in the Health Office. Any questions can be answered in the Health Office.
Yellow physical form may be picked up in any school Health Office or down-loaded from the District by clicking on the following links:

Health Office Approval for Participation in InterScholastic Athletics Physicals are provided for students:
- participating in interscholastic activities
- needing working papers
- needing school physicals for grade 10 or new students
The green physical form may be picked up in any school Health Office, from the Directors' Office or downloaded by clicking on the links above. The Health History side of the form must be completed, signed, and dated by a parent/guardian for the student to have the physical at school. If you have a special student who may need clearance for the Selective Classification contact the ATHLETIC DIRECTOR, Mr. Tatham at CNS HS. All appropriate forms must be completed by the parent and presented to the physician at the physical. Remember --this is for a few very select student-athletes.
