Spring Programs
SCHOOL PHYSICALS FOR FALL EXTRA-CURRICULAR ACTIVITIES AND WORKING PAPERS WILL BE OFFERED AT CNS ON THE FOLLOWING DATES: Physicals will be offered in the CNS HS Health Office on Thursday, May 27th for boys and girls. The yellow physical form may be picked up in the Health Office. The completed Health History side must be completed and signed by your parent/guardian----then you must sign up in the Health Office. Any questions can be answered in the Health Office. If you have any questions, please call Beverly Hill, RN @ 218-4103.
